HEALTHPLUS MEDICARE ADVANTAGE PRESCRIPTION DRUG PLAN

The HealthPlus Senior plan is a Medicare Advantage-Prescription Drug plan with a Medicare contract. Medicare has approved the HealthPlus Senior plan for membership beginning January 1, 2007 through December 31, 2007. The HealthPlus Senior plan is available in Genesee, Lapeer and Shiawassee counties. This contract may be terminated by HealthPlus at the end of the year or by Medicare if the plan does not meet regulatory requirements. You would be notified at least 60 days prior to contract termination and provided information noting your coverage options.
