Welcome to the HealthPlus Online Tools Section! In this section, you will find items available to your HealthPlus covered employees. These items are:
- HealthQuest Profile (HQP)
- HealthQuest Profile (HQP) a unique personal health assessment available to HealthPlus members over the age of 18. The HQP provides members with individualized information that can help them make positive lifestyle changes. The assessment is available through our secure HealthPlus website, as well as on paper if they are without Internet access.
- For employer group participation HealthPlus will provide you with a personalized aggregate report if thirty (30) or more employees complete the HealthQuest Profile.
- Personal Health Record (PHR)
- Personal Health Record (PHR) is an online resource that allows HealthPlus members to create, store, and manage personal health information including medications, doctor visits, and lab results all in one central location. This information is accessible to them anywhere, anytime the need arises through our secure HealthPlus website. The PHR is strictly confidential and is only accessible by the member.
- My HealthQuest
- My HealthQuest is HealthPlus’ easy-to-use, online health information resource offering health news, personalized risk assessments, health quizzes and calculators, drug references, self-care centers for improving your health, daily health tips and more.
